Since I really know nothing about any of the other options I voted for Pinoko. I think in any case she's probably the only one for him at heart seriously.
And as for her growing up. I'd say that's technically up to the writers, but I gather that Tezuka himself always traditionally liked her to be the size she is. So whether his company would ever choose to break that tradition now that he's gone, well we'll just have to wait and see.

Oh that's an easy one. Kiriko hands down.
I believe that Black Jack x Pinoko is rather freakish myself (age aside, it's still loli) and I also believe their personalities go horribly together. I disdain her possessiveness and her frequent temper tantrums.
